ILLINOIS PHYSICAL, DENTAL, & IMMUNIZATION REQUIREMENTS

PRE-KPhysical exam before FIRST DAY OF SCHOOL: 4 Dtap, 3 Polio, 1 MMR, 1 lead screening (Benson, Minonk, Washburn zip codes must have blood test- finger stick is adequate), 3 Hepatitis-B, HiB series or one after the age of 15 mo, Chickenpox vaccine or history of disease, Pneumococcal series or 1 shot if started between ages of 2-5 years, and the doctor should address the diabetes and TB sections of the physical. 

KindergartenPhysical exam before FIRST DAY OF SCHOOL:  5 Dtap, 4 Polio, 2 MMR, 1 lead screening (Benson, Minonk, Washburn zip codes must have blood test-finger stick is adequate), 2 Chickenpox shots or history of disease, and the doctor should address the diabetes and TB sections of the physical.  All students must have a vision exam by an optometrist by Oct of that current school year.  Dental exam due by May of that current school year. 

2nd gradeDental exam by May of that current school year.

6th gradePhysical exam before FIRST DAY OF SCHOOL:  all updated immunizations, diabetes & TB screening.  ALL STUDENTS ENTERING 6th GRADE NEED THE T-DaP shot, Meningitis shot, proof of 2 chickenpox shots or history of disease, & Hepatitis-B 3-shot series (probably already completed as an infant) due before FIRST DAY OF SCHOOL.  Doctors should check for scoliosis between now and 9th grade.  Dental exam by May of current school year.  The Gardasil vaccine is now available to prevent HP-Virus which has a strong link to cervical cancer (not required).

9th gradePhysical exam before FIRST DAY OF SCHOOL:  all updated immunizations, including, proof of  2 chickenpox shots or history of disease. The doctor should address the diabetes and TB sections of the physical and check for scoliosis.  Dental exam due by May of current school year.  

12th grade:  Students entering 12th grade need proof of 1 MENINGITIS shot after the age 16 years turned in before the first day of school.

** THE STATE REQUIRES PARENTS TO SIGN AND DATE THE HEALTH HISTORY SECTION!!  ** May fax health forms to 309-923-7508- “Attention Nurse”

STUDENTS NOT TURNING IN REQUIRED HEALTH FORMS WILL NOT BE ALLOWED TO START SCHOOL- Please turn in by the last day of Registration.  

Physicals may only be done by an MD, DO, or PA (nurse practitioner), NOT A CHIROPRACTOR.   Physicals are valid for sports for one calendar year.  

Sports physicals are valid for 13 months. Every student in a school sport must have a valid sports physical on file. 

To those applicable, religious objection sheets are required at Pre-K, K, 6th, and 9th.  

New students transferring in from out-of-state will need new physicals and eye exams.  

***Physicals and eye exams are valid if completed within one year of the first day of school.  Dental exams are valid if within 18 months prior to May of the current school year.
